Enquiries and complaints
Claims and grievances, including complaints, may be submitted by the policyholder, the insured person or the beneficiary under the insurance contract to TUnŻ „WARTA” S.A.:
- in writing to the following address: PO box 1013, 00-950 Warsaw 1;
- electronically via a form on www.warta.pl/reklamacje;
- by telephone at 502 308 308;
- at every TUnŻ “WARTA” S.A. branch, with a letter delivered in person or orally for the record;
- to the agent in the following forms:
- in writing – when submitted in person at a Branch of the Bank during the Branch’s business hours, or sent to the following address: Citi Handlowy, Departament Obsługi Reklamacji i Zapytań Klientów (Complaint and Customers’ Enquiries Service Unit), ul. Goleszowska 6, 01-249 Warszawa (Warsaw);
- orally – by telephone or in person for the record during the Client’s visit to a Branch of the Bank,
- electronically – to the Bank’s email address, via Citibank Online after logging in, in the tab “Contact us”. Current contact details for submitting complaints are available on the Bank’s website (www.citibank.pl).

TUnŻ „WARTA” S.A. is subject to supervision exercised by the Polish Financial Supervision Authority. Notwithstanding the provisions of GAIPT&C, the Insurer will comply with its disclosure obligations as provided for in the currently applicable legislation.

Benefit payment
If there has occurred an event covered by the insurance policy, the beneficiary shall submit an application for benefit payment directly to the Insurer. To that end, the beneficiary will fill up the form provided by the consultant of the Bank’s branch or the insurer – at any branch or online at www.warta.pl.
An application for payment of the benefit will be deemed filed on the date the application is received by the insurer. For detailed rules on submitting a claim for benefit payment and on benefit payment to be made by the Policyholder go to the GIT&C or to the website www.warta.pl.

Important Information
WARTA Ochrona is an individual life insurance of TUnŻ “WARTA” S.A. Details of insurance, such as rules of benefit payments as well as limitations and exclusions of Insurer's liability, can be found in the General Terms and Conditions for the Individual Life Insurance WARTA Ochrona (hereinafter: GIT&C), available at Citi Handlowy branches and on the website: www.warta.pl.
The insurance coverage may be enhanced to include additional insurance policies listed in the GIT&C. The sum insured and additional insurance premium are determined on an individual basis.
The Insurer may refuse to provide a benefit in cases described in the General Terms and Conditions of the Individual Term Life Insurance “WARTA Ochrona”, for example if suicide is committed within 2 years after the agreement was signed.
The Bank acts as an insurance agent carrying out agency activities on behalf of Towarzystwo Ubezpieczeń na Życie “WARTA” S.A., and is entered in the register of insurance agents maintained by the Polish Financial Supervision Authority. The Client may check the Bank’s entry in the above register via the website: https://rpu.knf.gov.pl. The Bank represents several Insurers in the same branch of insurance – and will provide additional details upon a Client’s request. The scope of the Bank’s responsibilities includes, in particular, informing the Policyholders of the insurance terms and conditions and of the rules of conclusion of the Contract.
